Waiver & Release of Liability

Assumption of Risk, Release of Liability & Waiver

By registering with Future Starss Soccer League ("the League"), I, on behalf of myself and/or my minor child(ren), acknowledge, understand, and voluntarily agree to the following terms:

1. Inherent Risks. Participation in soccer and related athletic activities involves inherent and unavoidable risks, including but not limited to physical contact, falls, collisions, overexertion, use of athletic equipment and facilities, and the actions of other participants. These risks may result in personal injury, illness, disability, permanent impairment, death, or property damage.

2. Voluntary Assumption of Risk. I understand and accept that these risks exist and I voluntarily assume all such risks on behalf of myself and/or my registered minor child(ren), whether or not caused by the negligence of the League or its affiliates.

3. Release of Liability. In consideration of being permitted to participate, I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE Future Starss Soccer League, its owners, organizers, directors, employees, volunteers, coaches, referees, sponsors, vendors, and any affiliated parties from any and all claims, demands, damages, losses, costs, or liabilities of any kind arising out of or related to participation in any League activity, whether caused by negligence or otherwise.

4. Parental/Guardian Authorization. I certify that I am the parent or legal guardian of the registered minor participant, that I have full authority to sign this waiver on their behalf, and that the participant is in adequate physical condition to participate.

5. Medical Consent. In the event of an emergency, I authorize the League and its representatives to seek and consent to emergency medical treatment for the registered participant.

6. Photo & Media Release. I grant Future Starss Soccer League the right to use photographs or video of the registered participant taken during League activities for promotional purposes, without compensation.

7. Physical Waiver & Player ID. I understand this online acknowledgment does not replace the requirement to sign a physical copy of this waiver in person. A player ID will be issued only upon completion of the in-person waiver signing.

8. Severability. If any provision of this waiver is found to be unenforceable, the remaining provisions shall continue in full force and effect.

I have read and agree to the Waiver and Release of Liability above. I understand that I must also sign a physical copy of this waiver in person at the league facility or at my child's first game or practice in order to receive their official player ID.
